Has anyone any recommendations on: Android TV Box for US, UK Channels incl EPL
Did you have to pay a annual subscription fee for the premium channels
there are a lot of boxes out there to choose from and a lot of shops down at Sim Lin Sq selling them as well as online vendors so it would be good to get some recommendations based on your experience.

No but most of them appear to be committing piracy via a range of technical means

If you want to watch Netflix in hd, get the Mii or nVidia. Devices need to be certified by Netflx to play higher than than 480p (which looks like ass IMO). I made the mistake of buying a generic one called Mecool M8s pro L. Apparently they used to have Netflix certification but they lost it by the time I got my box. And nothing else works very well on it. I should have ponied up a little extra for brand name box.
