Have you ever entered Singapore with different passport or Name

I had entered Singapore in 2015 with old passport which got expired.
Now I am entering Singapore on travel Visa, with new passport wih correction in name as compared to old expired passport. Visa agent in Bangalore says, I should furnish name change affidavit, which I feel is no the right way. while changing name in new Passport, we do all he excercise of Affidavit and advt in news paper. why should we do it again while applying for travel visa with the same name as in new passport.
if anybody in the forum have come across such situation, please advise.
