You folks were right. It was a mistake by the HR. Both employer and employee CPF contribution is capped to $5000 salary.

Just an update. The HR threw in an extra condition: employer will only pay the 16% capped at salary of $5000, while I (employee) will have to pay 20% of my full salary (no cap of $5000 for me).

hellboy_sing: This option was not offered by my HR. Are all employers bound by law to offer all 3 options? If yes, maybe my folks are hiding it to reduce their burden. Should I ask for it?

HR says full rate means employer pays 16% and employee 20% Graduated rate begins with employer 4% and employee 5% in first year and so on.
